One Valid ID Required For Fingerprinting

Examples of accepted identification:

State issued drivers license or ID card
United states passport
United States active duty military ID card
Retiree, or reservist military ID card
INS I-551 resident alien card
INS I-766 employment authorization card
United States tribal ID card

We Provide FD-258 Fingerprinting Cards

If you have your own FD-258 card, please bring it to the appointment. *Do not fill out or sign the card prior to your appointment.

If your company or organization requires additional forms for fingerprinting, please bring them to your appointment, we are not able to print forms.
